Rising Ohana’s CIC is now accepting referrals for young people, families, and parents with complex needs. To refer someone to our service, please complete the referral form below.
We accept referrals from:
– Local authorities and social workers
– Health professionals
– Educational settings
– Community organisations
– Self-referrals (for certain services)
How we use your information
Rising Ohana’s CIC will use the personal information provided on this referral form to assess the referral, understand the needs of the individual or family, contact you where appropriate, and provide or arrange relevant support and services.
We may collect sensitive information, including information about health, disability, SEND, wellbeing or safeguarding, where this is relevant to the referral or the support required.
Referral and ordinary service records will normally be retained for up to two years after our involvement or support has ended, unless there is a safeguarding, legal, contractual or other justified reason for keeping the information for longer.
We will not sell your information or share it for unrelated purposes. Information may be shared where necessary with relevant professionals or organisations involved in providing support, where you have agreed to this or where another lawful basis applies. Information may also be shared without consent where this is necessary to protect a child or adult at risk, comply with the law or respond to a serious safeguarding concern.
