Specialist Family Support, Assessment & Outreach Services
Rising Ohanas provides tailored, holistic and trauma-informed support for young people, parents and families presenting with complex or additional needs.
We work collaboratively with local authorities, NHS services, social workers, health professionals and other partner agencies to provide person-centred, strengths-based interventions that respond to the individual circumstances of each family.
Our approach recognises the relationship between parenting capacity, neurodivergence, cognitive needs, emotional wellbeing, environmental factors and wider social circumstances.
Support is informed by individual assessment and delivered through structured, flexible intervention plans, with regular reviews to monitor progress, identify emerging needs and adapt provision accordingly.
Who We Support – Ages 16 to 28
Rising Ohanas works with young people, expectant parents and parents aged 16 to 28 who may benefit from additional, tailored support.
Our provision includes antenatal support during pregnancy and postnatal support following the birth of a baby. We work with individuals and families at different stages of their parenting journey, including those navigating their transition into parenthood, developing parenting confidence, or requiring additional advocacy, wellbeing or community-based support.
Support is responsive to assessed needs and individual circumstances, including neurodivergent, cognitive or other additional needs, and may be delivered through parenting interventions, practical outreach, holistic assessment and coordinated work with referring professionals.

We offer structured parenting interventions designed to strengthen parenting confidence, practical abilities and family relationships.
These may include antenatal and postnatal support, parenting skills development, routines and boundaries, child development, emotional regulation, bonding and attachment, and support for parents with additional learning or neurodivergent needs.
Interventions are adapted to individual communication, comprehension and learning requirements.

This may include:
- One-to-one key-working and family support.
- Emotional wellbeing and psychosocial support.

We work collaboratively with relevant professionals to promote engagement, strengthen family resilience and reduce barriers to accessing support.
4.Duration, Monitoring, Review & Referrals
Interventions may commence with an initial six-week period, with the option to extend support where an ongoing need is identified.
Duration is determined by the complexity of the family’s circumstances, assessment findings, agreed objectives and progress achieved.
Support plans are reviewed regularly and adapted where appropriate. Professional communication, safeguarding procedures, case recording and outcome monitoring form part of the agreed delivery arrangements.
We welcome enquiries and referrals from local authorities, NHS professionals, social workers, early help teams, Family Hubs, education settings and community organisations.
Following receipt of a referral, we discuss the presenting needs, assess suitability, agree the scope of intervention and establish relevant reporting and review arrangements.
Where commissioned, the service can be tailored to the requirements of the referring organisation.
